knows but you.  Clothes can hide exercise-resistant fat and sagging,

Tummy Tuck (Abdominoplasty)

A Tummy Tuck, or Abdominoplasty, is a cosmetic procedure that
offers enhanced body contour and a firmer, flatter abdomen for women
and men who have loose abdominal skin and fat concentrated in the
abdominal area.

Abdominoplasty removes flaccid abdominal skin and tightens muscles
that have been separated and weakened by pregnancy or weight gain.  
This procedure is particularly effective in women who are finished
having children.  It removes most, if not all, of the stretch marks caused
by childbirth.

In the traditional “full tummy tuck” procedure, we create a new belly
button and tighten the muscles from the lower chest area to the pubis.  
The scar is usually from hip to hip.  Liposuction can be performed at
the same time to create a nice contour.

Thanks to advances in Tummy Tuck surgery, there are many variations
both to the design of the incisions and the technique itself:
•        A partial abdominoplasty, or “mini tummy tuck” — When the
amount of loose skin is minimal and the excess fat deposits are located
below the navel, a short horizontal incision is all that is necessary.
•        Endoscopic abdominoplasty — another technique for minimizing
scars and may be useful when patients have only a mild degree of
excess fat and muscle laxity.
•        A circumferential tummy tuck — this is usually for someone who
has lost a great deal of weight and fat (usually after gastric bypass
surgery) and now has loose overhanging skin all around the body.  In
order to create a nice contour, we need to extend the scar all around
the waist line.  This allows us to lift the outer thigh and buttock, creating
a smoother nicer look. Also the muscle is tightened and the new belly
button is created.

Surgical Overview
•        Length: 2-3 hours.
•        Anesthesia: General (asleep).
•        In/outpatient: Outpatient or impatient (one night stay).
•        Side effects: Temporary soreness, swelling, bruising, and change
of sensation in the lower abdomen.  The incisions from the procedure
will heal and fade overtime.  It is important to realize, however, that the
incision lines will be permanently visible.  In some instances, they will
eventually be only faint lines.  Certain individuals may have incision lines
that are more noticeable.  Fortunately, the incisions for your
abdominoplasty are usually in locations concealed by most bathing suits
and undergarments.
•        Risks: Bleeding, infection, risks of anesthesia and scarring.
•        Recovery: Back to work: seven to ten days.  One can shower
the following day and resume activity as they feel comfortable. A
compression garment is worn for approximately two weeks. Drains are
usually necessary for one to two weeks.
•        Duration of results: If weight remains stable, results are usually
permanent.

Liposuction

Liposuction improves body shape by removing exercise-resistant fat
deposits with a tube and vacuum device.

Common locations for liposuction include: chin, neck, upper arms,
abdomen, buttocks, hips, thighs, knees, calves, and ankles.

This procedure can be performed using the super wet technique, in
which targeted fat cells are infused with an epinephrine solution before
liposuction to reduce post-operative bruising and swelling.
For larger volumes of fat or for fibrous body areas, ultrasound-assisted
lipoplasty (UAL) may be used.  UAL is a technique in which an
ultrasound probe is inserted beneath the skin to “liquefy” the fat before
it is suctioned.

Dr. Goodwin also employs Laser-assisted Fat Removal (SmartLipo)
which can be performed in the office on appropriate patients.

Surgical Overview
•        Length: 1-2 hours or more. UAL 20-40 percent longer than
traditional liposuction.
•        Anesthesia: General (asleep).
•        In/outpatient: Usually outpatient.
•        Side effects: Temporary bruising, swelling, numbness, soreness,
burning sensation.  Super wet: temporary fluid drainage from incision
sites.  UAL: larger incisions for cannula.
•        Risks: Asymmetry.  Rippling or bagginess of skin.  Pigmentation
changes.  Skin injury.  Fluid retention.  Excessive fluid loss leading to
shock.  Infection.  UAL: thermal burn injury caused by the heat from
the ultrasound device.
•        Recovery: Back to work: one to two days. More strenuous
activity: two to four weeks. Full recovery from swelling and bruising:
one to six months or more.  Use of super wet technique or UAL may
decrease post-operative bruising and swelling.  A post-operative
garment is usually necessary.
•        Duration of results: Permanent, with sensible diet and exercise.

Fat Injection

Armlift (Brachiplasty)

Another area of concern for those patients having significant weight loss
is excess tissue in the
upper arm area. No matter how much exercising or toning you do, that
skin will stay there unless it is surgically removed.  
The goal is to improve contour in the upper arm.  Dr. Goodwin’s
technique also aims to minimize the appearance of the scar through
incision location and appropriate fat removal.

Surgical Overview
•        Length: 1-2 hours.
•        Anesthesia: general (asleep).
•        In/outpatient: usually outpatient.
•        Side effects: temporary bruising, swelling, numbness, soreness,
scarring, tightness with arm movement.
•        Risks: Infection, bleeding, wound breakdown, scar hypertrophy,
asymmetry, risk of anesthesia.
•        Recovery: Sometimes drains 1-2 days. Resume normal daily
activities within 1-2 weeks.  Full recovery in 6 weeks.  Post-operative
garment for 4 weeks.
•        Duration: Usually permanent if weight remains stable.


Thigh Lift

is excess tissue in the medial upper thigh, just below the pubic area. No
matter how much exercising or toning you do, that skin will stay there
unless it is surgically removed.
