The term “plastic surgery” includes both cosmetic and reconstructive
surgery.

Cosmetic surgery is performed to enhance or reshape an otherwise
normal physical feature of the body, thus improving appearance and
boosting self confidence.  When a cosmetic procedure is entirely elective,
it is customarily not covered by health insurance.

Reconstructive surgery is performed to repair a physical abnormality of
the body.  Such abnormalities can be caused by birth defects,
developmental defects, injuries and accidents, infection, tumors, or cancer
and other diseases.  Reconstructive procedures are typically conducted
with the goal of enhancing function, and in some cases, to establish a more
natural appearance as well.  Most health insurance plans provide
coverage for reconstructive surgery, although the types and amounts of
coverage provided may differ.

In certain situations, insurance coverage for plastic surgery may be
provided at the discretion of the insurer.  This can apply to either
reconstructive or cosmetic procedures and is contingent upon an
individual’s specific circumstances.  For example, breast reduction
(reduction mammoplasty) might be covered insurance if the excessively
large breasts (macromastia or gigantomastia) are determined to be a
physical impairment.  If it is for cosmetic improvement only, it will likely
not be covered by health insurance.
